Basic Purpose
Responsible for all clinical components of resident health services within the specific regulations for assisted living; to support the mission and philosophy of BSI, Inc.
Major Duties & Responsibilities
Performs chart reviews to verify accuracy and completion of required documentation.
Coordinates health services for all residents; reports significant changes to families and physicians.
Takes initiative to ensure timely resolution of identified resident care issues.
Ensures Resident Care Attendants are delivering care according to standards set by DH & BSI, Inc.
Duties include but not limited to:
a.
Determine duties which can be safely and appropriately performed by unlicensed personnel.
b.
Make written assignments for Resident Care Attendants.
c.
Provide supervision of duties performed by Resident Care Attendants.
d.
Initiate disciplinary action as outlined by Broadway Services, Inc.
Employee Handbook when necessary.
e.
Participate in employee performance evaluation process.
f.
Review Person-Centered Service Plan (PCSP) with Resident Care Attendants.
g.
Identify training needs for Resident Care Attendants.
h.
Monitor Infection Control practices in the Assisted Living Community.
i.
Work directly with the Director to access the abilities of residents in Assisted Living.
j.
Foster positive community relations.
Fulfils all duties to direct/provide resident care as it relates to Assisted Living.
Duties include but are not limited to:
a.
Observe the condition of the resident, record any pertinent changes in the resident’s condition in the medical record and notify the attending physician, responsible party, and the Director of Health Services.
b.
Monitor vital signs as ordered by physician and document in the medical record.
c.
Assess and orient new residents upon admission to the community.
d.
Re-assess residents quarterly or as needed.
e.
Participate in the service planning process including: assist in scheduling of service plan meetings; assessment, implementation, evaluation, revision, and documentation in the medical record.
f.
Assist and monitor medication use as ordered by the attending physician.
Update PCSP as needed with regards to medications.
g.
Monitor medications stored in the Medical office and in the resident apartments for appropriate labeling and expiration dates.
h.
Assist with monitoring for timely response by Resident Care Attendants to emergency pendant calls.
i.
Follow community policies and procedures in the event of a resident’s death/emergency.
j.
Assist residents with ADL’s and IADL’s as required.
k.
Educate residents and family members as needed; report concerns to the Director of Health Services and Assisted Living Director.
l.
Monitor dining room meals; identify residents with loss of appetite, swallowing impairment.
m.
Assist Resident Lifestyle Director in getting residents to and from activities.
n.
Assist with emergency situations according to policy and procedure.
o.
In addition to the required dementia training, must attend at least 12 hours of in-service annually, including ADL’s and IADL’s.
Follows federal, state and local regulations, as well as administrative policies of BSI, Inc.
Duties include but not limited to:
a.
Maintain a current license to practice as a Licensed Practical Nurse in the State of Louisiana.
b.
Manage and maintain records of controlled substances in the Assisted Living Community in a manner that is compliant with local, state and federal laws.
c.
Document legibly all monitoring and observation of resident in the resident’s medical record.
d.
Ensure the staff is familiar with all fire and disaster plans.
Minimum Qualifications
1.
Graduate from a State accredited school of Practical Nursing.
2.
Currently licensed to practice as a Practical Nurse in the State of Louisiana.
3.
Continuing Education as required by the Louisiana State Board of Examiners for Licensed Practical Nurses.
Excellent physical and mental health.
Ability to adapt and execute orders of physicians and Director of Health Services within limitations of education background/licensure, and with an understanding of the cause and effect of such action(s).
Ability to communicate effectively with co-workers, residents, families, physicians, and the general public.
