*New Visions of Life Care Services is seeking a Direct Care Supervisor/Facilitator *
Must be at least 21 years of age or older at the time the offer of employment is made.
Also, have a High school diploma or general equivalency diploma (GED).
Must have a valid drivers' license, reliable transportation, and able to make visits after and/or before office hours, if needed.
Be able to work well independently and as a part of a team.
Have good written and verbal skills.
* Make unannounced visits to evaluate the staffs abilities to perform assigned duties;
* Ensure each staff has annual performance evaluations;
* Assist with clients request for services or change in services the date of such request to the appropriate authority;
* follow emergency medical training while attending the client;
* subsequently report any medical emergencies to the supervisor, the provider or others, pursuant to the provider policies and procedures;
* If needed, document and report any suspected abuse, neglect or exploitation of clients to CPS, APS, or EPS on the date of discovery, and as required by law; also, assure that the clients health and safety isn't being compromised.
* Train staff on daily documentation such as progress notes, progress reports, service logs, and EVV.
In addition, perform weekly oversight check to assure services are being rendered based on daily documentation of services, preferably on Mondays.
* Knowledgeable on how to work LaSRS
* Ensure that the status of clients being reported on progress notes and/or progress reports.
* Complete Critical Incident Reports per guidelines i.
e.
2 hours upon discovery verbal report to support coordinators, write report within 24 hours, and follow-up within 72 hours.
* Complete the ISP, back-up staffing plan and emergency plan for all Children's Choice Waivers, New Opportunity Waivers, and only back-up staffing plan and emergency plan for Community Choice Waivers that's generated through OAAS forms.
* Ensure that all documents related to the services are turned in on a timely base, so the client won't have an interruption in services according to their CPOC or POC.
* Annually, ensure that the provider's agreement is discussed with client, signed, and place in their personnel file.
* Monthly ensure the staff is completing their fire drills.
* Check home binders when going into the client's home to ensure all necessary components are in the binder.
* Ensure that all clients are staffed according to their ISP, POC, and/or CPOC
* Occasionally, as needed, act as a back-up staff if staff isn't available in unforeseen circumstances to provide services and not place the client at risk for services not rendered according to their CPOC, ISP, and/or POC.
