*Responsibilities and Duties*
* Provide individuals in-home assistance with basic personal care such as toileting and grooming activities, bathing, hair care, and assistance with clothing or exercise.
* Provide individuals in-home assistance with medications/health care.
* Provide in-home assistance with nutrition such as menu planning, grocery shopping and meal preparation.
* The DSS is responsible for all documentation related to the accomplishment of these duties.
* Provide in-home assistance with other personal care activities as assigned.
* Transport individuals to and from grocery or personal shopping; bill paying; doctor appointments; dentist appointments; social service or social security appointments; banking, etc.
* Be a member of the Individual Service Plan (ISP) team by attending ISP meetings and instructing individuals on self-help skills as documented in the plan of care; observes individual progress/behavior and documents data; guides individuals in accomplishing ISP objectives by demonstrating techniques and reinforcing good performance/discouraging inappropriate behavior.
* Participates in program operation by documenting behavior, activities and progress on data sheets; completes various reports and forms; maintains order among individuals by promoting and reinforcing helpful and friendly behavior.
Assist individuals in daily maintenance of living areas; identify maintenance/repair needs to Team Leaders, Team Supervisors and Administrative Director and Office Staff for assistance; maintain a safe, clean, hazard free environment.
* Have knowledge of independent living skills, "Becoming Independent.

* Evaluate and determine individual's strengths and needs.
* Attend staff meetings and other in-service training offered by Advance Home Care Outreach and workshops outside of Advance Home Care Outreach.
* Be familiar with emergency procedures such as fire/tornado.
* Keep all information confidential.
* Be a "role model" for individuals served by Advance Home Care Outreach and a representative of Advance Home Care Services, LLC.
* Conduct and document tornado drills quarterly and monthly.
NOTE: In most cases people being served will need more than assistance.
The Direct Support Staff will provide light housekeeping (general cleaning), grooming, laundry, nutritious menu planning and shopping FOR the person in order to ensure the good health and general well being of the person receiving services.
*Qualifications and Skills*
High School Education or equivalent.
MUST be able to pass background check.
This person must be able to operate a vehicle; have a valid La driver's license in good standing and a good driving record for the last three years, and the background check must come back complete and clear.
Proof of current vehicle liability insurance.
Ability to lift at a minimum of 25 pounds.
Ability to observe and record behavioral data and daily task.
Ability to communicate clearly with staff and consumers through use of telephone and other verbal means.
Ability to follow necessary procedures in case of fire/tornado or other emergency situation.
Ability to successfully complete required in-service training for this position (particularly the 40 hour DSS training) as evidenced by written and skills test with update every 6 months.
Knowledge and experience of working with people with developmental disabilities is helpful.
